Vermont Creates Program to Train Community Health Workers
As a part of their State Implementation Program IV grant, the Vermont Action Coalition worked to promote interprofessional practice in home health settings. They created a Community Health Worker (CHW) curriculum geared towards recent American citizens. At the time of the grant ending, 18 participants completed the program.
