Transforming Primary Care Delivery Systems
In 2019 The Colorado Center for Nursing Excellence was awarded an innovation grant to transform and improve primary care delivery systems by developing nurse leaders in primary care who can improve health outcomes and increase patient access.
This work was made possible through support from the Center to Champion Nursing in America, a joint initiative of AARP Foundation, AARP and the Robert Wood Johnson, Foundation
The workshop was designed for nursing leadership, clinic administrators, human resources and professional development staff, medical directors, clinic operations directors, and team members interested in innovation.
The Cultivating Nurses in Primary Care workshop was hosted by Clinica Family Health, a grant partner and Federally Qualified Health Center.The underpinnings for the curriculum was the 2016 Macy Foundation Conference Recommendations to transform primary care by utilizing RNs at the top of their scope in primary care settings. Participants learned how to improve clinic systems so that RNs were fully utilized leading to improved patient experiences, quality and access.
