Robert Wood Johnson Foundation (RWJF) asks grantees and contractors about the data and results of any public opinion survey or research survey it funds to ensure it adheres to RWJF standards. This is an editable form to be completed and sent to SIP@aarp.org after a survey is fielded; survey and results must be approved before the publication of any survey results.
